I declare that the information given in this form is correct and that I am duly entitled to complete this form on behalf of the participant. To the best of my knowledge there is no reason, either medical or otherwise why the participant should be unable to complete the workshop. I enclose a deposit of £50 and understand that the balance of £80 (8-19 year olds) or £20 (4-7 year olds) is payable no later than 1st July or in cash on the first day of the workshop. I understand there is a discount of £20 on all siblings aged 8-18. All payments are non-refundable should I later decide to cancel. I give my consent to any emergency medical treatment necessary during the activities and thereby authorise staff to sign on my behalf any written form of consent required by the hospital authorities should medical treatment be necessary. In accordance with statutory principles governing fair and lawful processing of personal information, I consent that my details and the details of others referred to on this form may be held by The Big Act and may be used to inform me of related products and services in the future. I understand that my details will not be passed on to any other organisation. The Big Act reserves the right to restrict admission at it’s own discretion. I acknowledge that photographic material involving my son/daughter and taken in connection with The Big Act may be stored on computer and used by The Big Act free of charge and without prior consent from me.
